Transitional Care

 

Leaving the hospital should feel like a relief, but for many patients and families, it comes with new worries. The days and weeks following a hospital stay are often the most critical—individuals may feel weak, unsteady, or uncertain about discharge instructions, and families may not always be available to provide the constant support needed. Without the right care, recovery can be delayed, or worse, complications may lead to readmission.


That’s why Pointes of Care offers specialized Transitional Care services. Our goal is to make the move from hospital to home as safe, comfortable, and stress-free as possible. By providing extra support during this important time, we help individuals regain independence while giving families peace of mind.

Our transitional care services include:

  • Hospital-to-Home Support – Coordinated assistance on the day of discharge, ensuring a safe trip home and a smooth adjustment.
     
  • Medication Management – Reminders and oversight to ensure prescriptions are taken properly and on time.
     
  • Personal Care Assistance – Help with bathing, dressing, grooming, and mobility during the recovery period.
     
  • Follow-Up Care Coordination – Support with scheduling and attending doctor’s appointments, therapy sessions, and follow-ups.
     
  • Meal Preparation & Household Tasks – Nutritious meals, light housekeeping, and organization to create a safe, comfortable home environment.
     
  • Recovery Support & Encouragement – Gentle reminders and motivation to follow discharge plans, therapy exercises, or activity restrictions.
     
  • Family Relief – Stepping in when loved ones can’t be there, providing reassurance and continuous support.
